GAME DAY T-shirt Reservation Form -- See the T-shirts HERE!

* Please submit one t-shirt reservation per size, per person.
* If you would like to order more than one shirt of each color (Black = home, White = away), please submit a separate form for each order.
* ATHS will sell a single shirt of either color for $10, but the "2 shirts for $15" must be 1 black & 1 white. Thank you for your understanding.
* Please bring/send payment to the Front Office. If you are making multiple reservations, you can combine payments. If sending a check, make it payable to Apache Trail High School.
